Joint Commission issues revised
standards
The Joint Commission has posted new
and revised standards effective as of
January 2018.
Included are:
* Life Safety Code revisions that address
topics such as testing of
emergency lighting systems and inspection
and testing of piped medical
gas and vacuum systems.
* Medication Management revisions
that ensure the standards reflect
evidence-based practices and quality
and safety issues.
* Healthcare-associated infection revisions
that address National Patient
Safety Goal 7: " Reduce the risk of
healthcare-associated infections " for
hospitals, critical access hospitals,
and new requirements for nursing
care centers.

Decreasing airborne
contaminants
Wearing long sleeves with gloves while
prepping a patient in the OR decreased
large-particle and microbial shedding
compared with prepping with bare arms,
this study finds.
A mock patient skin prep was performed
in three different ORs. A longsleeved
gown and gloves or bare arms
were used to do the preps. Particle
counters were used to assess airborne
particulate contamination, and active
and passive microbial assessment was
done with air samplers and settle plate
analysis.
OR B had decreased 5.0-µm particle
sizes with the use of sleeves. ORs A
and C had decreased total microbes
only with the use of sleeves.
Though there was no difference in
the average number of total microbes
for all the ORs, the use of sleeves specifically
decreased the shed of Micrococcus.
Long
sleeves may decrease airborne
contamination while performing the skin
prep, which may lead to decreased surgical
site infections, the authors say.

CMS updates Hospital
Compare site
The Centers for Medicare & Medicaid
Services (CMS) has updated data on
its Hospital Compare website and will
update the Overall Hospital Quality
Star Rating using a new methodology
that will give more hospitals one or five
stars.
The Overall Star Rating has been revised
to use an enhanced methodology
to assign ratings to hospitals, based
on Technical Expert Panel recommendations
and public input. The agency
intends to update the Star Rating twice
per year, in July and December.

PMI
common after noncardiac
surgery
Perioperative myocardial injury (PMI) is
a common complication after noncardiac
surgery (occurring in 1 of 7 patients),
and it is associated with substantial
short- and long-term mortality
(9% and 22%), this study finds.
This analysis included 2,018 patients
having 2,546 surgical procedures.
PMI occurred after 397 of the
surgical procedures.
Only 6% of patients (24 of 397) with
PMI had typical chest pain, and 18%
(72 of 397) had ischemic symptoms,
which indicates a major difference from
spontaneous myocardial infarction.
The major differences between PMI
and spontaneous myocardial infarction
mandate individualized treatment strategies,
the authors note.
