Public reporting of C-section rates
As of July 1, 2020, the Joint Commissio
will begin publicly reporting hospitals
with consistently high C-section rates on
" Quality Check, " using hospital data reported
during CY 2018 and 2019.
The Joint Commission began requiring
accredited hospitals to collect and
submit data on the perinatal care (PC)
cesarean birth measure PC-02 in 2010.
In 2017, 25% of reporting hospitals
had rates of more than 30%, leading the
Joint Commission to reexamine its position
of publicly reporting these rates.

nets cost savings
In the first 2 years of Medicare's Comprehensive
Care for Joint Replacement
(CJR) program, there was a modest reduction
in spending per procedure without
an increase in complication rates,
this study finds.
Comparing costs associated with
280,161 joint replacement procedures
in 803 hospitals required to participate
in the CJR program with 377,278 procedures
in 962 hospitals that did not
participate in the program, researchers
found an $812 savings per hip- or
knee-replacement procedure, or a 3.1%
reduction in costs.
The program also was associated
with a 5.9% reduction in use of skilled
nursing care after hospitalization, and
complication rates did not increase.

Infection control in anesthesiology
The Society for Healthcare Epidemiology
of America (SHEA) on December 11
published a new expert guidance on " Infection
prevention in the operating room
anesthesia work area. "
Key recommendations include:
* Increased hand hygiene performed,
at a minimum, before aseptic tasks,
after removing gloves, when hands
are soiled, before touching the anesthesia
cart, and when entering and
leaving the OR.
* Use of double gloves during airway
management, so one layer can be removed
when contamination is likely
and the procedure is moving quickly.
* High-level disinfection of reusable
laryngoscope handles or adoption of
single-use laryngoscopes.
* Disinfection of high-touch surfaces
on anesthesia machines, keyboards,
monitors, and other items
in between surgical procedures, as
well as use of disposable covers
and re-engineering of the work surfaces
to facilitate quick decontamination
in a short window of time.
* Use of syringes and vials for only
one patient, and disinfection of injection
ports and vial stoppers before
access.
The authors encourage investment in
research to better understand infection
control problems posed by the anesthesia
work station and to develop design
improvements that reduce infection risk.

Gallup rates nurses highly
An annual poll from Gallup found nurses
rated the highest among professions
in the US for their honesty and ethical
standards for the 17th straight year.
A total of 84% of respondents rated
the honesty and ethical standards of
nurses as " very high " or " high. "
Physicians came in second with
67% rating their honesty and ethical
standards as " very high " or " high, " and
pharmacists came in third at 66%.
Members of Congress, telemarketers,
and car salespeople had the majority
of " very low " or " low " ratings.
Healthcare workers, including
nurses, physicians, and pharmacists,
continue to earn the highest ratings
for their honest and ethical standards,
says a December 20, 2018, report.
