OR leaders feel the pain of the opioid crisis
he OR isn't exempt from the effects
of the opioid crisis. Identifying
patients taking opioids is part
of OR clinicians' responsibilities, and
they also play an important role in preventing
opioid addiction.

" We have patients who have opioid
use disorder [OUD] and neglect to
tell us, " says Mary Jo Steiert, BSN,
RN, director of nurses at Lowry Surgery
Center in Denver. " That's a problem
because they could potentially overdose
here or after they go home. "
According to Phillip Eichenholz, MD,
president and founder of NorthStar Anesthesia
in Dallas, there are 3 million
new, persistent opioid users thanks to
inpatient exposure following surgery.
In the 1990s, clinicians were told
to focus on pain control and not worry
about possible addiction, which didn't
occur with short-term opioid use postoperatively.
" That was wrong, " Eichenholz
says. Providers also felt compelled to
relieve all pain so patients would report
being satisfied with pain management
on surveys used to determine
reimbursement (sidebar, p 13).
But the landscape has changed. The
opioid crisis has sparked responses
from regulatory bodies, and hospitals
and ambulatory surgery centers (ASCs)
are taking a closer look at how opioids

companies have imposed increasing
penalties on healthcare
facilities in recent years for readmissions.
And with good reason: High readmission
rates greatly increase costs
and sometimes signal poor patient
safety practices.

In 2015, roughly 2 million patients
were readmitted, costing Medicare
$27 billion, according to the Centers
for Medicare & Medicaid Services, and
$17 billion was spent on readmissions
that were potentially avoidable.
Knowing which patients are at high
risk for readmissions and optimizing
their health before surgery are key
to reducing readmissions. Numerous
studies support the merits of these
practices, and tools such as the American
College of Surgeons National Surgical
Quality Improvement Program's
