Study: Cause of blood clots in
some COVID-19 vaccine patients
A new study looked at the causes behind
some patients developing deadly
blood clots following the administration
of certain COVID-19 vaccines. The findings
were published in the journal Blood
on October 26.
Previous studies had shown that
patients with Vaccine-Induced Immune
Thrombocytopenia and Thrombosis
(VITT) produce antibodies that stick to
a protein called Platelet factor 4 (PF4),
creating a cluster of molecules called an
immune complex. This complex led to
clotting. For this study, researchers took
blood taken from healthy donors, as
well as serum and plasma from patients
with VITT, and discovered how PF4 triggered
the production of the small cells
known to cause clotting.
The researchers say the findings will
allow them to develop new drugs to inhibit
this mechanism and prevent deadly
clotting and may help them identify ways
to prevent clotting seen in other diseases,
in addition to guiding future vaccine
programs.

Forbes Advisor ranked the best and
worst states for healthcare based on 24
metrics and four key categories: healthcare
access, outcomes, cost, and quality
of hospital care, Forbes October 13
reports. According to the article:
* Georgia was the worst state for
healthcare based on factors such as
residents without health insurance
(12.63%), kidney disease mortality
rate (18.87 deaths per 100,000
state residents), and stroke mortality
rate (44.27 per 100,000 residents).
* Minnesota was the best for factors
such as low death rates of flu and
pneumonia (7.03 per 100,000 state
residents) and kidney disease (7.37
per 100,000 residents) and quality
of hospital care.

Joint Commission ORYX
reporting requirements
The Joint Commission on November
1 announced the release of the 2024
ORYX performance measure reporting
requirements, which will be effective
starting on January 1, 2024. In the new
requirements, acute care hospitals and
critical access hospitals must join The
Joint Commission National Healthcare
Safety Network (NHSN) Group.
It noted that sharing NHSN data will
allow it to include more recent data in
Accelerate PI dashboards as opposed to
publicly available CMS data, which can
be between 18 and 24 months old. The
data will also more readily identify quality
improvement opportunities.
Hospitals must also allow The Joint
Commission to access de-identified
data on five CDC NHSN hospital-associ*
Colon and Abdominal Hysterectomy
Surgical Site Infection
* Facility-Wide Inpatient Hospital-Onset
Methicillin-Resistant Staphylococcus
aureus Bacteremia.
The Joint Commission NSHN group
will share information on joining NSHN to
hospitals in late 2023 and early 2024.

AAAHC:
Updated toolkit for safe
surgery with obese patients
Rates of obesity have tripled over the
last 60 years, and some 70% of US
adults are overweight or obese.
The Accreditation Association for Ambulatory
Health Care (AAAHC) released
its revamped Ambulatory Surgery Considerations
for Obesity and Obstructive
Sleep Apnea Toolkit, ASC Focus October
2023 reports. It provides updated guidelines
to ensure patients with obesity
and/or obstructive sleep apnea remain
safe throughout surgery.
The toolkit includes a flowchart as
an assessment resource prior to surgery
for healthcare providers, taking into
consideration a patient's BMI, sedation
levels, and preexisting conditions.
It also includes updated considerations
for obese patients who previously contracted
COVID-19.
