APS investigates abuse, neglect, or financial harm to vulnerable adults over 18, offering

Burial Assistance
This program provides financial assistance to help pay for reasonable and necessary funeral
and burial expenses for low-income Boulder County decedents.

Long Term Care Medicaid
This program offers eligible individuals comprehensive services, including in-home and
community-based assistance, the Program for All-Inclusive Care for the Elderly (PACE), and
extended nursing home or hospital care. It aims to support those with various mental and
physical health needs, helping people remain in their community.

Medicare Savings Program (MSP)
MSP helps people with limited income and resources pay for some or all of their Medicare

WellMind Connection
WellMind Connection serves as a central point for connecting residents to appropriate
resources and accessing the right behavioral health care. This referral and communitybased
service connects residents to resource navigators and the Crisis Assistance and
Treatment (CAT) team in English and Spanish.
