How in the heck do y'all schedule for a single visit crown and are done in two hours or less? I can prep, build up, retract and have ready to scan usually in about 30 minutes from the time the patient sits down. I don't think there's any way we could have it scanned, designed, milled, ﬁred and etched back in one hour. Our scan usually takes about 10 minutes, the design is about 10-15 minutes, the mill takes another 12-15 minutes, then we have a ﬁring (for e.max) of about 24 minutes, then etching, ﬁtting and adjustments (if there are any) and cementation. It's almost two hours of time other than prepping, and sometimes longer than that. It's just not adding up for us. What on earth are we or aren't we doing? What does your appointment breakdown look like? We have taken to releasing the patient and having them return in an hour. That puts us closer to two-and-a-half to three hours, start to ﬁnish. I would love to get it under two hours, otherwise we will go back to scanning, temporizing and delivering. We are about to upgrade to the NEVO, so I'm sure that will help, but I can't help but feel that I'm off on something for the milling or ﬁring. I'm taking longer than everyone else. It could also be the designing, because I have my assistants do it. I usually can have them done much quicker, but I also need to be in the next room with another patient. I'm not frustrated with the system, my patients love it and I have good ﬁtting crowns. I just want to be as efﬁcient as possible with it. We don't have an overﬂow operatory to put patients in, just two operatories at the moment. We can look at ﬁtting out the third one, but even then I'm not sure how to schedule that. ■ TMC29ZX Member Since: 07/29/11 Post: 1 of 33 FEB 12 2014 Your ﬂow and everything sounds good. The new system will change your scanning into minutes. My ofﬁce manager can do it in two minutes! I've cross trained extensively. But do you have the speed tray and setting for your oven? We can pin, glaze and bake one in 17 minutes (14 of that is actually in the oven). If you do have the speed setting, make sure the hood is closed while you're prepping and pinning, and just when you're ready to bake it, open the hood and hit start on your oven. If your hood is open the time will be extended because it won't start closing unless it's hit a certain temp. But we are routinely in the two to two-and-a-half hour range. Have you tried changing the sprue to keep your restoration on an I12 block? And make sure your burs on the mill are relatively sharp. ■ dmdatwork Member Since: 08/16/08 Post: 2 of 33 FEB 14 2014 I guess we don't have the speed tray? Any chance you could post a picture of that one? I think they use the standard setting (P1) on the oven, which is 24 minutes. I think the only other programs we have are the correction ﬁring or the CAD program. That would actually make a huge difference because the pining, glaze and baking probably takes the better part of 35 minutes. We actually just got the (very late) memo on the I12 blocks. We had pretty much been doing everything with C14 blocks with e.max and you're completely right, we were going through burs left and right. I think that will help too. I think if we could get it down to that time range we would be much happier and not feel like it's taking too much time in our day. Thanks for your help!
