Across the board, patients described the procedure as pain free. They take Tylenol or Advil immediately after the surgery and most took nothing else for pain after the single dose. My experience: my patient had Miller Class 1/ 11 recession on # 3 , # 4 and # 5 ; # 3 has been extremely sensitive for years. When we told him he'd be sore after the surgery, he said it couldn't be any worse than what he's been living with. The surgery went very smoothly. We had two hours scheduled for him but I don't think I was in his mouth for more than a half hour. He reported some discomfort when the anesthetic was wearing off and he took Tylenol and Advil combo. When he woke up this morning he Pre-op had utterly no pain. The Essix Nightguard we made for the lower arch was the only thing bothering him. This is j ust a modiﬁed semilunar coronally-repositoned ﬂap and in some cases it will work, but when the patient such as this case has a 9 -10 mm bony dehiscence over the exposed MB molar root, which never had any bone to begin with, what are we accomplishing with this less-painful surgery? All we are doing is moving some tissue with a dissolvable membrane. I have my clinical doubts as to the long-term efﬁcacy of this procedure. I would not do this procedure on myself, let alone a patient. These cases will not probably hold up long term. Sometimes less pain means less gain. Someone please prove me wrong. Y ou (not you speciﬁcally but the techniq ue itself, as not to critiq ue the operating person) are not increasing any attached tissue, you cannot see the roots to get them super-smooth, you are placing a resorbable collagen membrane inside a pouch that will resorb and do nothing more than this. R egular tunnels or ﬂap surgery with pap-sparing incisions give you so much better control of the root surfaces and help secure the connective tissue or membranes to the CEJ regions. These pinhole techniq ues do not allow this. The whole marketing with this techniq ue is less pain. I, like many periodontists, have done thousands of CTG s full-mouth and Puros and tunnel surgeries and very rarely get a call from someone who is hurting, plus we have properly modiﬁed and smoothed the roots to an ultra-smooth.
