Two teeth become extruded and displaced after the patient hits her face against concrete. Will endo
treatment be required?

A patient slipped next to the pool and hit her face against the concrete. Teeth #8 & 9 were
extruded and lingually displaced. These were the only mobile ones. She went to the emergency
room first, where they took a CT and told her no bones were broken. I set #8 & 9 back into
place, heard a little pop that freaked me out when pushing #9 back into the socket, the same
kind of sound you sometimes hear when a tooth pops out of a socket during extractions. Did a
class IV filling on #8 and took care of any decay. Then I bonded a splint on the linguals of #6-11.
Can you all check out these X-rays and let me know if you see any root fracture? I see a little
something, but am really hoping they are accessory canals. I told her we'll see how things go
with the splint, but she may lose #7-10. Called her today and she says she's feeling good.
What are the chances that she'll get to keep her teeth? What code should I use to bill for
the splint?
Sorry for the angulation here. This is before setting her teeth back into place, so it was
difficult getting the sensor in there.
After setting her teeth back into place...

I say the odds are good that she might get to keep her teeth. She will likely need endo, but
time will tell. Great job! ■

I think endo should be done on these teeth as soon as you have some stabilization. Any
chance of internal resorption needs to be minimized. ■
