Reverse Prep
Townies discuss different techniques for prepping crowns.

Does anyone on here use the reverse-prep technique with prepping for crowns? Also, do you
have a bur block that is made for it? I am thinking of trying it out. ■

Seen DiTolla's video on it. Never tried it myself. Not real big on having to buy specialty burs
for a crown prep. ■

Heard him recently at a San Mateo Dental Society presentation. Tried it a few times. Have to
make it a routine. It works if you give it a try. You don't have to buy the name-brand diamonds.
You can buy similar-size diamond burs from private dealers, i.e., Dentalree in Las Vegas or
Microcopy, and save some money. You can buy separate bur holders as well. Hope this helps. ■

I bought the block and did the technique a bit. I find I can accomplish the same thing more
quickly using guide pin diamonds to rapidly prep a chamfer in one step, rather than in two with
the DiTolla technique. Guide pin diamonds also quickly and reliably prep a perfect chamfer. ■

I have recently started using bullet-shaped and pear-shaped ultra-fine diamonds (yellow
stripe) on all crown preps. The margins never looked so smooth and the crowns never sat better.
They barely cut, so you almost burnish a margin with them until they glide all around the tooth.
Very few companies make them-I get them from Dentalree.com.
I'm looking for a similar bur for my CEREC preps, but no one seems to make a flat-end taper
with a head width of 18+...and thinner heads ditch the margins. ■

Wouldn't that pear bur create a slight undercut? ■

Never heard of guide pin diamonds, Jay. Will have to look those up! To the OP just use a
round bur and whatever axial-reduction bur you like. Those occlusal reduction burs are hella
handy though. I don't have them anymore and miss them. ■

Those depth-cutter burs in the kit are expensive! I've used the system many times; I prefer
"conventional" preparation for the most part. ■

I used to do reverse crown prep when I did non-CEREC C&B. I just used an appropriate-size round bur to start my F and L margins. No special bur needed and then just used my
chamfer diamonds. ■
