Machined Collar New to Fixed Hybrids - Questions/Advice Fixed Hybrids If you've played with guided surgery, you know that there are a couple of requirements: You need cone-beam data (DICOMS) and CAD/CAM data (STL model). The cone beam has become pretty routine in most implantology offices but the CAD/CAM has always been problematic. The reason is that there's a high cost of entry ($15K¬-$60K) and often closed architectures that keep you locked into a single system and don't allow export of STLs. While I've been out of work and had time on my hands, I've been playing with this technique to reliably get an STL model from purely cone-beam data that is accurate enough to build a guide on ... and it works! There Fig. 1 are a couple of companies that already offer this, like the newest Carestream unit, but it's an expensive add-on. This will allow you to do it with almost any unit. [Fig. 1] Here's a patient and what you're seeing is cone-beam data. These are virgin teeth and yet there is still some scatter on the scan and it's not accurate enough to build a guide on this and expect it to fit. And if the patient has any metal in the mouth ... forget it. Fig. 2 [Fig. 2] So what we have to do is plan our case on the cone-beam data (implant position, emergence, etc.) but then merge an STL model to this that we'll construct the guide on. Why? Because the CAD/ CAM data is much cleaner and gives you a well-fitting guide. [Fig. 3] Here you see that merge. Fig. 3 Fig. 4 [Fig. 4] And here you see the implant position that was previously planned and the clean STL model ready to fabricate a guide. [Fig. 5] Here's the guide fabricated and ready for 3-D printing. So what's the big deal? To get that STL, you had to either send this to a lab to be digitized and lose several days to turn the case around, or have an open-source optical scanner that gives you an STL and that's a lot of money.
