Class 2 Composite Recurrent Decay
After noticing decay beneath a recently placed Class 2 composite,
this Townie wonders how to prevent this from happening again

Eighteen months ago, I placed Class 2 resins using a product called Beautiful Flow on #13
and #20. The BW image showed decay under the bottom of the proximal box only of #20,
which then went around occlusally to the proximal dentin. I am thinking there was no seal at
the bottom of the box. What are you clinicians using to seal all proximal cavosurface margins so
recurrent decay does not happen? Not sure how to attach the X-ray image. Thanks in advance
for any help! ■

Given it has only been a year and a half, perhaps you didn't excavate all the decay in the first
place? I use a small layer of flowable under my restorations, makes me feel better about complete
curing against the tooth. ■

I do not do Class 2 with only flowable. Way too unpredictable, in my experience, and rarely
looks good at recall. If you want to go flowable in a Class 2, do the Belvedere technique: some
flowable in the bottom of the prep and fill in with packable. Makes for pretty nice margins and
helps with white lines. ■

Possibilities:
1. Bonding issue. Check bonding protocol.
2. Saliva contamination while bonding/filling.
3. Light issue. Check/calibrate your curing light. Or operator error. Maybe your assistant was
curing the wrong tooth. If so, the composite will be stained brown.
4. Check the composite. See if it cures properly in vitro.
5. Open contact/food trap.
OK, I'm out of ideas. Let us know if you find out the cause. ■

If you bulk-filled this and cured it with a standard curing light, it almost certainly is uncured
in the base. If you have that crazy sapphire curing light, it may be still uncured in the bottom. ■
