Put a vertical cut fi rst and don't go to the full width of the crown so we don't damage the lingual tissue. Aim for half to 2/3 of the crown and then section tooth in two-medial and distal half. Then put horizontal cuts and separate coronal sections of both mesial and distal halves. Obviously, it always doesn't go 100% as planned, and have to cut down one of the bigger pieces into smaller pieces sometimes. But prefer to make tooth smaller than making a hole bigger. We have another provider in our organization who tries to take even full bones in one piece and sometimes I get to see his patients postoperatively and I see a signifi cant difference in postop swelling, infl ammation and discomfort.
