I wouldn't be so quick to write off patients who decline treatment. After a few recall cycles,
they'll begin trusting you more and more once you become a familiar face. They're most likely
listening to your recommendations and just need time to process them, even when it feels like
they're just blowing off your treatment plans. ■

I think with these types of patients, you really need to purchase an intraoral camera. If you
are seeing broken-down teeth consistently, a photo will resonate with patients much more than
any radiograph (if you have computers, that is). My practice recently bought out an older doc's
patients who have had some patchwork dentistry and you would be surprised at the amount of
MODBL amalgams converted to BU/crowns through intraoral pictures. Patients really appreciated
it and knew they were getting a higher level of care. ■

First, I'm happy to see your thread, as I'm following in your footsteps: buying a small cashonly
practice from an older doc, closing in December.
My thoughts: You have doubled collections. You mentioned staying FFS " for as long as
possible. " The fact that you've doubled your collections tells me that you should stay FFS forever.
You have options. I'd just go slow. You have the collections, you have the money, you have profi t.
That is the most important thing.
Make changes slowly. Win over the patients and the staff you want to keep, let the others walk.
You have six months before any sort of " reality " will hit you and if you are seeing HYG pts now,
winning them over now, then they will be back in six months and your problem takes care of itself.
You paid for a lot of patients that you may lose, but your termination letter gave you a lot of
patients you didn't pay for. So I wouldn't worry about that.
Stay FFS. Stay profi table. The nice thing about FFS is that you can do less and make more.
Instead of expanding, I'd concentrate on focusing on the best patients in the practice. Let the
$100 repair people walk, lose 1/3 of the patients, and be left with the 2/3rds who care about their
teeth (or 1/2) and will pay for crowns/SRP/etc.
Remember, one crown at $1,000 is worth 10 of the " repair " patients. I'd rather see two

legacy-type practices. They have a lot of opportunities, but they also come with very
underperforming employees. I've found that it is nearly impossible to turn the employees around.
I've had a couple do it, but it took a lot of work to get them to where I wanted them to be. ■
