Do you guys think that extracting this tooth would
have made the situation worse?
I have just included the X-ray of the tooth in 2022. ■

Stop getting hung up on the radiographic image
with radiolucent fi ndings. Th at is a late fi nding in
most oral cancer cases. Most cancer in the mouth is
squamous cell carcinoma. Th at is an epithelial disease
meaning it starts from the soft tissues. Gums,
cheek, tongue, fl oor of the mouth mucosa, etc. I do
think there are case reports of primary intraosseous
SCCA, but it is rare. Meaning the cancer started in
the bone and moved outward. X-ray imaging would
be the only way to fi nd that early.
But for most cancer people will come across in
their careers, the cancer will start as a soft tissue
lesion. Red, white, papillary vs ulcerative, etc. but
will look diff erent than the adjacent soft tissue. By
the time you see bone destruction on a radiograph,
that cancer has progressed beyond its origin site and
is now destroying adjacent structures and becomes
stage four. ■
fi rst molar. Th e pre-op wasn't so obvious, but the
post-op changes were obviously something bad. Pt
had to have her jaw resected. Nice older lady, the sad
part of the story was that her husband had recently
passed away from cancer and her daughter was
dying of cancer at the time. She ended up surviving
for a few years, but her quality of life went downhill.
Don't beat yourself up about having extracted that
tooth. ■

It grew larger after the extraction because it's cancer.
It will grow regardless.
Th e reason extracting teeth sitting in cancer
is bad is because of the way the soft tissue heals.
Th e cancer grows down into the socket. Now there
is bone invasion (if there was not already) which
makes it stage four. ■

case earlier in my career. Except it was a lower left
