I agree with the previous two posts. Smooth
interprox after prepping, and make sure there's draw
with the adjacent. Lab can't give you broad contacts
if there's no space to do it. The other thing might be
your temp. Try to get it to ideal. n

So, should we always flatten the adjacent teeth as
much as possible with a fine-diamond bur? n
not, I don't touch a crown for adjusting. I used to
literally adjust my crowns to flat tops with no anatomy.
My embarrassment of them via photos is what
prompted me to seek out help, and I was glad to have
switched labs. Even my staff was like " OMG, those
new crowns look so good! " I still have to adjust, don't
get me wrong, but it's such a small percentage that I
almost laugh when I do because I get upset I have to
adjust one.
My two cents: Find a lab that will help you work
through issues. Not worth the hassle of adjusting
every crown you have to cement. n

Don't use a bur to polish the interproximal; that
can be really hard to get just right. Use a disc, such
as a Sof-Lex medium. n

I've been in the same spot-was getting to the
point I dreaded the crown seat. Margins always
great, but contacts so inconsistent. I just hated telling
a patient their tooth wasn't right and had to go
back to the lab. I second what Tuan said: Cerec has
made my seats an absolute breeze. Minimal adjustments,
no stress. n
our retiring CDT. So easy to add a small amount of
add-on porcelain on a full zirconia crown to tighten
a contact. No more sending back to the lab. I schedule
45 minutes for a crown seating, and can add a
contact if I have to and still be done in the allotted
time most of the time. Sure beats reanesthetizing,
recementing and another appointment. Definitely
worth looking into. I've probably added contacts to
about 25-30 crowns over the past year. If it's even
slightly light, I add and tighten it. Paid for itself multiple
times over just in the last 12-18 months. Entire
process of adding on takes me about 15-18 minutes,
including the fire time. n

Have you talked to your labs, and if so, what did
they say? I had the same problem-tons of adjusting.
I got an iTero to solve my problems but, again, tons
of adjusting. Called the lab, little to not much info,
nothing changed or noted for me to change.
Decided to switch labs. Not as bad but similar
issues; called lab. Lab discussed issues to try and,
bingo, we got things solved where most times than
shortfill

It's more than likely your temporary that is causing
issues. Have them give you a static cast with
tight contacts. This works for 3D-printed models too
(but not as well). Make them adjust them to where
there is a snap on the static cast with the floss.
You are probably getting material in the interproximal
that is wedging the tooth apart when they chew
on the temporary. n
