Thanks, and great presentation, MHD! I like the idea of the two-denture plan upfront. I
have had lab pour up a working model from my initial pickup and only denture at the surgery/
delivery before-but do like the idea of having the second on hand, especially when they call
and say they " think " they may have broken a piece on their appliance. Thanks again for posting,
always generates a lot of discussion where I pick up a few tips or tricks I haven't thought of.
I always do these cases with my oral surgeon because a full-mouth case like this is a long-ass
appointment, even when everything goes perfect. But, man, are you tired after one where things
had to be adjusted/modifi ed on the fl y! I admire you guys doing it all by yourselves. ■

MHD: Nice as always. Could you share a little of your thought process on how you determined
the incisal edge of the dentures without a try-in? Looks like you nailed it, and a few pearls
on how you got there could be of help. I see you measured around 5mm of reduction. Thanks
for posting. ■

If the prosthesis provided on the day of surgery is accurate, then you don't need a try-in
because you have an exact duplicate of that also picked up on the day of surgery. The lab scans
this duplicated denture that can screw onto the master model and has an exact position of where
the teeth need to be. The bar is designed backwards based on where the teeth need to be, and
then the lab making the restoration just duplicates the tooth position and fi ne tunes occlusion.
I used to use Voco Quickup and it was fi ne, but I recently switched to Stellar DC acrylic and
it is excellent to work with. Sets fast. Is incredibly hard. No primer needed, is a dual cure, so can
light-tack things when desired. ■

Yes, but how did you determine the amount of bone reduction and the position of the incisal
edges for the immediate temporary? Great case here. ■
Thanks. The incisal edge position of her existing failing bridge was nice, so we just asked the lab
to maintain. Bone reduction is just working backwards from there to make sure you have 15+ mm. ■
When you do the two denture pickups, is that after you have sutured? If so, how do you

keep the sutures from getting tangled in the pickup material, and what material do you use? ■

I mark the locations of the MUAs with bluebite material before suturing and then hand off
temp copings on the MUAs.
Each temp coping has one trimmed piece of rubber dam with a hole punched that is placed
over/around the coping to prevent pickup material from going everywhere. ■

While my assistant is opening up the holes, I suture the arch and then place my fi rst set of
