Thank you everyone for the help. I attempted the
case yesterday. I ended up having to place a 3.7x13
instead of the planned 4.2x13 because I was getting
too close to #10 even though I started the osteotomy
mesial to try to get the larger implant. I also ran out
of time, so I didn't have a chance to place a custom
healing abutment so instead I grafted the buccal
gap, placed CollaPlug and a stock HA.
I took a scan after the implant was placed and I
realize now that I was quite a bit more lingual than
I originally planned. I think it's because I started the
osteotomy too high in the socket (about 5 mm down
the wall.) As a result, the lingual wall especially near
the crest was much thinner than I would have liked.
Will this be okay? I can still remove the implant and
just graft as an option.

I'm not sure if this was said before. Please plan your
restoration first and plan to place the implant 4 mm
apical to the future crown margin. You can also use
neighboring teeth as a guide during surgery to measure
from platform to the CEJ of the tooth next door.
It also depends on how you will temporize. If you
are burying the implant then I don't think you need
to drill past the apex in a case like this. Just enlarge
to a drill size 0.5 mm smaller than the implant apex.
If you can place a 3.8 or a 4.x2x10 and just engage
the apex you will be fine, and it will come out of
the cingulum for screw-retained crown. Place the
implant at least a 2 mm subcrestal. In a case like
this I would temporize with an Essix retainer.
Take your time with the extraction and preserve
the buccal bone. Place the bone graft in the buccal
gap and close with the d-PTFE membrane on top.
This is not a difficult case if you know what you
are doing, and with proper planning. Also keep in
mind a direct implant is not self-threading, so once
you get primary stability leave it alone. Otherwise,
you'll get a spinner. n
