Help With Immediate Denture for PPOs

I was looking for input on this and wanted to see
how everyone is doing it. Bonus if you have a removable
lab that isn't pricey but is quality.
When I've done immediate dentures for insurance
patients, I usually say they'll need two soft relines
between the delivery of the immediate denture after
extractions and the hard reline six months later.
I would say this works for 70% of my patients.
About 30% whine and act like I did something
wrong or delivered a denture that I found in a bag.
(These patients usually refuse the relines until I offer
a soft one for free.) With that initial complaining and
problems, I usually don't get to the hard reline.
I've read some providers offer a throwaway immediate
and have the patient sign one of those HIPAA
release forms from insurance, then deliver a conventional
denture.
How are you participating in insurance with
patients who need full-mouth extractions, then
dentures? I'm looking for a more bulletproof protocol.

Travis, what codes are you charging out or not
charging for the " trial " denture? I guess from your
statement that you're charging an immediate denture
fee based on their insurance? 

Simple answer? Plan to make two dentures and tell
them the first one is a trial. This change has greatly
improved patient care and expectations for me. They
no longer expect the first one to be perfect. 
bill an interim, will they then pay for a second denture
a few months later? Complete denture, obviously
bill to insurance. My office has not been billing
the interim to insurance and the patient pays that
one out of pocket. Curious if we can bill them both
and expect payment for both. 

Do you charge for two dentures? Full fee? 

We won't submit this to insurance. It's a low-cost
prosthetic. Patients pay for this out of pocket. If they
refuse, they can go elsewhere. They will pay per soft
reline/tissue conditioning. Some patients will tough
it out with no relines at all through the healing
period (not recommended). Some want a new one
every month, and bony changes can justify it. At
