length and diameter for the particular implant system that you're using-they are not all the
same!
Yes, you absolutely could use a plastic temporary abutment. It might even be better than this
carrier technique. The only thing I don't like so much with the temporary abutments is that they
are designed for cement-retained restorations-something I avoid like the plague. The carriers
are free and easy to use. I used to only use the plastic temporary abutment, but we haven't had
one in the office for about five years now.
One thing I would not recommend for a single unit is to use a Balance Base abutment
and a temporary cylinder. These are like the Nobel multiunit abutments, and the hint's in the
name-they are simply not stable as single units.
I absolutely agree with what you have to say about primary stability. With Ankylos, you have
to have enough that you can immediately load the implant, but not so much that you break the
carrier (like I did yesterday on another case). That requires a finesse of touch that I was sadly
lacking yesterday, but which is usually fairly easy to assess with a little experience. I wouldn't
have loaded this if it was a spinner.
I do tend to go through the entire drilling sequence for these immediates. I forgot to post the
preoperative CBCT scan that showed I couldn't go longer than an 11mm without coming out
through her nose. Remember, the crown is completely out of occlusion.
This lady came to me as a self-referral, so we will be restoring the case and not her own
dentist. I still do end up doing a lot of immediate loading cases when the referring dentist is
restoring then. I'm not sure what's the difference between putting a sulcus former on the implant
to shape the tissue, or putting a temporary crown on the implant to shape the tissue. The
referring dentist still gets to make the definitive crown.
I actually don't know if the tooth was painful. I didn't do the initial consultation. I just
know she has seen a specialist endodontist elsewhere who recommended that it be removed. I'm
just a surgeon and will defer to my smarter colleagues who peer down microscopes all day over
this. ■
