The goal here is to show a recent case I completed and hopefully along the way answer some questions I see frequently on the boards. This gentleman showed up as a new patient and was told he wasn't a good candidate for implants. He was thrilled to hear he was an excellent candidate-he paid in full for his surgery on the way out and scheduled for two weeks later. Overview of proposed treatment. Notice the radiographic markers on the scan. These are just small dots of fl owable composite placed on the patient's existing denture (which he hates). These markers allow me to have a radiographic guide, which I can then translate to a surgical guide. Notice the excellent AP spread of the implants. By placing in the fi rst molar positions and one in the midline, this prosthetic is fully supported from front to back. This isn't always possible, but it should be the goal whenever it is possible. Surgical guide. All I did was duplicate the existing denture in acrylic in my offi ce and then cut holes where I want implants to emerge. Cheap. Easy. Fast. Allows for predictable, prosthetically driven freehand surgery. No large lab bill or fancy guides that take three weeks to make. Implants placed. Excellent stability.
