I have a patient who lost #8 and #9 because of
resorption (and we are working on hygiene as well).

I typically make them myself. If the screwretained
gets temporary abutments and build up/
bond with composite, they can look pretty nice if
done with care. I typically take impression at the
time of placement and place temps at the time
of uncovering. Can tweak the emergence profi le
very easily. ■

Th anks for the response! I fi gured this was one
way but was concerned things would be harder
because these are placed relatively deep.
Do you take a preoperative impression and make
an Essix, cut access holes, block the intaglio of the
temp abutment with a cotton-tip applicator or
something, and just bulk-fi ll composite to get the
large initial shape of each restoration? ■

I don't typically do this, but I would like to use
provisional implant restorations to make sure I have
the gingiva shaped correctly before proceeding to
fi nals. Th e surgery was done well, and access seems
to be right through the cingulum so my provisionals
and fi nals can be screw-retained. Th is will also give
the patient time to see if he wants me to restore only
#8 and #9 and try to match his other teeth, or if he
wants to go big and do #5-#12 or #4-#13.
How are you guys doing this relatively " cheaply "
(from a lab bill standpoint) at all? Is there a way
to do this without using two sets of custom abutments?
Are you typically using PMMA crowns for
the set of provisionals? ■

You need the appropriate analogs to allow you to
add the subgingival composite for the correct contours
and polish. Easy procedure. ■
