Here's a less cluttered photo of the extracted tooth
and the provisional I made on cheap abutment.
Th e temp is Luxatemp and composite with GC Optiglaze
on the facial. ■

Great question. If there is enough tissue to just
trim it to create a proper emergence profi le, I'll use
a curettage diamond. In this case, I actually made a
small releasing incision palatal to the papilla. Th en I
created my interproximal contours and gently rolled
the papilla to the facial. Th e provisional held it there
and then it granulates in. ■

I used to do the wax-up/Essix/holes route; I still do
if I'm doing a bridge (with non-engaging cylinders).
Now, for a single unit, I take a metal cylinder (temp
abutment), carve it to be smaller than the fi nal tooth,
put pink block-out over the facial, then use my normal
aesthetic composite to freehand the tooth. Th en
I unscrew it, fi ll the area between the tooth and the
abutment platform with fl owable, smooth and let
it sit. I will then remove it and add or subtract the
composite to contour tissue after a few weeks. ■

What techniques are present to help a lab mimic a
temp custom abutment when fabricating the fi nal?
Townie " Bone&Porcelain " told me the best way to do
it, I asked my lab to do what he said and the directions
went over their head. Said it couldn't be done
the way I requested, then sent me a bulky crown
that pushed the margins apically. Ugh. ■

To the OP: Excellent placement. Good job. Maybe
a bit too apical, but beautiful symmetry and parallelism.
Lane, I missed your posts. Really nice results,
I am glad I'm back!
One question: I usually try to do the gingival contouring
at the time of exposure. Now that you have
exposed and have the rather narrow healing caps,
would you not have issues with trying to expand the
emergence profi le?
For academic purposes, how would you go about
enlarging the space for proper contour? Of course,
if there is a low lip line and especially that you are
doing both #8 and #9, you will have excellent results,

You could take the temp out, screw it onto an
analog, then scan it. In 3Shape, the lab can import
the scan as an " Additional Scan " and merge it with
the preop, which gets merged with the scan body
scan. Th ey then have the custom temp on the virtual
model as a guide. It should be the same in Exocad,
but I'm not 100% sure. ■
