John, I think you might be referring to my thread (Silver Diamine Fluoride Disaster!),
but that papilla recovered nicely-see the updated photo on that thread. I don't believe
there have been any other cases of significant effects on the papilla from SDF, but please
correct me if I'm wrong. That papilla incident sparked me to do a lot of research on the
subject, and I'm convinced the 'disaster' incident was a little more complicated than SDF
= papilla destruction. ■

I would recommend flossing and PreviDent 5000. These are definitely 'watch' incipient
lesions that could remain as such for years to come and they could even remineralize. I certainly
would not destroy healthy tooth structure in order to access questionable caries. If she were my
daughter I would monitor over time. I have had 'watches' for 25 years on patients. Imagine if I
had filled them back then what the status of those teeth would be today. ■

These are the type of pts that will jump office after you tell them they have cavities. They
have never had cavities and now the hungry new doc says she does. They end up with a doc who
will 'watch' it. Then the Google review shows up and everyone knows you are a hack. ■

I find that it solidifies trust and helps patients understand. They'll say, 'Oh, I trust
you, Doc." I'll tell them that I appreciate that but I want to help them understand what's
going on. ■

I wouldn't touch these unless something was visible clinically, say on transillumination.
(Or unless you repeat the X-ray and get a better look at it.) This is a person with a
minimal history of caries, and these lesions, while suspicious, are all questionable in my
opinion. ■

I am saying use the IO photography as a teaching aid. I believe that patients, in this mode,
especially need the ability to 'trust and verify.' A picture of brown decay in a tooth and the extent
of removal of hard tissue to eradicate it speaks volumes. This has never backfired in my office. It
of course depends on how you present it, verbiage, body language, etc. ■
