At my new patient exam, I dictate the findings directly to my treatment coordinator, who writes them down into the computer. The software does not put it into any type of cohesive report. I’m digital in the office except for treatment cards. Tried that. Didn’t work for me. I dictate to the treatment coordinator but I always write up a treatment plan on the white card before we ever start treatment. Yes, it is comprehensive and takes a bit of time, but we get very few questions and it is much appreciated to keep everyone on the same page and nice for reference. Internally – in the computer for us – we keep it pretty basic since we both have the same mechanics, wire sequence and treatment strategies. For example, a general treatment plan in the chart might look like the following: 1. RPE 2. Bond U/L 7-7 3. Band U6s w/TPA 4. Evaluate for bonding U2s or IPR L ant 5. Evaluate for grafting L ant post-ortho 6. Retain with HBP, L Essix 7. Now, I dictate diagnosis and treatment plan into a computer chart (time and date stamped). But out of curiosity, if you send a detailed letter to the dentist detailing diagnosis and treatment plan, but have it nowhere else in the chart, would you be covered in a malpractice suit?
