This 16-year-old female has been followed for a few years with serial cephs, and the past
three (taken about one year apart) have shown no mandibular growth. The parents are
strongly against surgery. I’ve been racking my brain. Anyone have any thoughts on a non-surgical approach to her case? Neither the patient nor the parents are concerned with chin
prominence or any non-dental facial aesthetics. My plan is to extract her first premolars (max
and mand), and set her up for two-jaw surgery. Any thoughts?
Sorry her pano is a physical X-ray that has not been digitized yet. Nothing exciting
except some 8s that will be extracted with the 4s. n

I would treat this case non-extraction if the parents decide to go with the surgical option.
I think you can correct the negative OJ non-surgically by extracting both lower 4s, but this
would make the lower incisors retroclined even more and this is no good. n

This is the most I’ve been able to do with lower bicuspid extractions. His wits were about
-5; your patient is -12!
