Invisalign Trays Hanging Off Posterior?
Any ideas on why this Townie's maxillary trays are not seating completely in the posterior?

Has anybody else been experiencing a problem with their maxillary trays not seating
completely in the posterior? I just went from tray #6 to #7 and now the maxillary tray is sitting about 4mm off the first and second molars on one side. And this has happened with
several other patients as well. Any ideas? n

It is completely unrealistic to change the trays every two weeks-tooth movement does
not occur that fast-it does not occur that fast in fixed appliances, and does not occur that
fast in removable appliances. Go back a couple of tray sizes, and have the patient wear the
trays at least a month. You will have less difficulty advancing tray size in the future. n

I notice the same thing in Essix retainers. I think it's the impression material pulling out
of the tray upon removal.
I agree. Three weeks with aligner trays. I bet if everyone started doing this, we would
look at Invisalign differently. n

Plus one... three weeks minimum has greatly reduced poor tracking and need for early
refinements. Rarely have the original poster's problem, though. Huh? n

I have not had any seating issues since I started using iTero.
My guess is slightly distorted PVS impression combined with poor aligner wear.
I start off patients with three weeks-once they are two months in-go to two weeks.
Once they are really compliant you can reduce to 10 days.
I use Acceledent for some cases and they are tracking perfectly with seven-day turnaround (but you have to keep patients really motivated not to cheat). n

I have seen this exact issue several times and we do iTero scans.
Usually the tray will fit completely after the patient wears it for several days. This makes
me think that the fit issue is due to the particular movement built into that tray. It may be
posterior movement, either space closure or space opening, which then does not allow the
tray to seat into the interproximals.
However, in a couple of cases, I have had to reorder that tray, which then fit fine. n
