How Best to Treat
This Problem?

A 32-year-old male
patient has a chief complaint
of wanting a broader smile
(notice how teeth are constricted in transverse), and
wants front teeth to overlap
(no open bite). He is adamant about not wanting surgery and does not want to change his facial
features. I gave him some alternatives other than surgery but wanted to see what others would
suggest before listing them here.
Note, he does have generalized recession along his cervical margins which he claimed is due
to hard brushing. He had ortho as a 15-year-old and believes teeth have since relapsed. Believes he
ended that treatment with his teeth looking fine. What treatment options would you consider? ■

Of course I will never get an ideal result and it has taken many aligners.
