message board How Would You Treat This Case? A Townie asks for feedback about how to proceed-extractions? uprighting? -and shows his results Fenrisúlfr Member Since: 02/25/09 Post: 1 of 19 Introduction A 31-year-old presented with a chief complaint of crowding, and some clicking and popping on opening. No pain. She does report history of TMJ locking during opening. Conclusion: I'll post progress and final records after a few opinions. ■ 3/9/2017 fesdds Member Since: 05/22/08 Post: 2 of 19 Just looking on my phone today, but it looks like a 4 Bi extraction case. I would go with 5s and bond U/L 7-7. It may also consider bonding 8s for max anchorage if that is doable in her mouth. ■ 3/9/2017 odell060 Member Since: 11/19/11 Post: 3 of 19 I was thinking ext of 5s/5s, partial space closure and retraction of lower 3s before engaging lower incisors, Class II mechanics, fi xed appliances, 30 months +/-. ■ 3/9/2017 Fenrisúlfr Member Since: 02/25/09 Post: 4 of 19 I like both plans and getting on the 8s is a great idea. I've banded 8s for anchorage and it works well. In her case, the TMJ locking open made access to the 8s challenging. ■ 3/9/2017 egbraces +1 for ext of U5s/L5s ■ Member Since: 01/22/10 Post: 5 of 19 Str8edge Member Since: 07/17/11 Post: 6 of 19 3/9/2017 Definitely 4 Bi's here. I'd probably go U4s/L5s for more retraction and easier Class II correction, but would probably work out fine either way. ■ 3/9/2017 22 MAY 2017 // orthotown.comhttp://www.orthotown.com
