What's your treatment plan for her? One option might be to remove two lower incisors with
the poorest prognosis and upper 2nd bi's or upper 2s (canine sub). The way things are, she is
unable to clean around the lower 3-3. Depending on her overall attitude, there might be some
benefit in ortho to improve bone levels and overall prognosis for the entire dentition. I have seen
some major improvement in motivation and care after improved lower alignment in some patients
... good that you have a perio on board. ■

I had thought about two incisors, but the two that look the worst are the LL1-2. That would
leave the LR1-2 severely tilted. ■

You could two stage it with extraction of the LL2, close space using the LL (stay off the LR1,
2) with light OCS from LR3 and power thread from LL3. Once the space is closed, extract the
LL1. As things unravel the LR1, 2 will drift by transseptal pull and you could eventually place
brackets and align them with .016 or .018 NiTi and SS. ■

If I did it this way, do you think it would be wise to not place the bracket on the LL1 in ideal
position and instead just drag/push its current tilted angle since it would end up being extracted? ■

I would attempt to idealize it, in order to move the root of the LL1 distally, since the goal
would be to allow as much drift of the LR1, 2. With the large span and flexible wires, things
should move easily with lower M:F ratios. ■

Get a CBT scan to evaluate the true bony picture. How about doing an intraoral scan and
send it to Invisalign or Insignia and do a diagnostic setup on more than one possible treatment
plan. Maybe just extracting one incisor in the lower arch first and treat the lower arch to start. ■

On cases like this I generally disregard ideal OJ/OB etc. I would not extract on the upper. Been
a long time since I did that much interproximal reduction but as Charlie has stated he will do as
much as 8mm. This would be an ideal case for this since her caries index is so low and there are
