This presentation of an impacted canine isn't that
rare, in my experience. And they can successfully be
brought in.
Seems early to be starting full ortho on this
patient. We would instead have all C's extracted,
have UL3 open uncovered on the palate, and place a
lower holding arch.
A CBCT would be helpful to better localize the

Look at where the root end is. Sometimes the root
The poorly centered pan doesn't help in assessing,
either! Trying to think anatomically. What is the
physical barrier? Lateral nasal fosse? n
will have miles to go to get in the normal position.
Reposition with surgery or extract. n

Second law of ortho: Two things cannot occupy
Could be that. But at this point, hoping for selfcorrection
is like reading a self-help book: It's a waste
of time. If you work with a skilled surgeon, have them
expose it and bond a gold chain to the cusp tip. Then
extract #H and put the gold chain down through the
socket and start erupting it on Day One. It's a cool
trick and would work great in this situation. n

have any chance, #4 must be moved out of the way.
To move the #4, you have to extract the " D. "
Fourth law of ortho: Just because something can
occasionally be done doesn't mean it should be done.
If #3 doesn't start to redirect after #4 has changed
position, consider extracting #3 and keeping the
long-rooted " C. " I like to think of myself as an orthodontist,
not an orthodeity. n
