message board Ectopic U3s Buccal to 2s in Mixed Dentition Townies debate which teeth might need to be extracted for this case CliveStaples Member Since: 10/13/14 Post: 1 of 8 Age: 10-1 Class I skel/dental Retruded incisors Maxillary crossbite with max transverse defi ciency Max 3s buccal to Max 2s I am thinking max RPE, then reevaluate for extraction of max Cs and Ds, then pray for a miracle without promising one. Very likely will need exposure and ligation of max 3s. In the meantime, I will teach him how to brush his front teeth. I think some would place 2x4 braces with MRT on U2s and OCS to open space for the 3s to drop ... but I'd rather not get involved in that way if I can help it. The 3s are clearly right on top of the 2s, at least on the right side. Yeah, I hate when that happens. Any tips? ■ 4/9/2019 pezortho Member Since: 10/27/11 Post: 2 of 8 Maxillary expansion followed by extraction of UCs for sure. As much I don't like 2x4s in Phase I, this would be a good case for it to align the upper laterals and move the roots out of the way. I think there's a good chance you can avoid future surgical exposure of the U3s. ■ 4/9/2019 CliveStaples Member Since: 10/13/14 Post: 3 of 8 zxzxzx Member Since: 02/28/03 Post: 4 of 8 christjl Member Since: 02/07/13 Post: 5 of 8 I appreciate the feedback, Pez! ■ Why are you hesitating to extract the upper Cs? Clear out the space while the 3s are high and they should resolve. ■ Obtain a limited-fi eld CBCT before any orthodontic tooth movement. Better to know where you do not want to move teeth. Maxillary expansion is another demonstrated method to prevent canine impaction. I would zxzxzx Member Since: 02/28/03 Post: 6 of 8 expand in this case due to transverse constriction and then remove primary canines if needed. ■ It is my understanding that maxillary expansion works for palatal canines. Does it also work for buccal canines? ■ 4/10/2019 4/10/2019 4/10/2019 4/10/2019 20 DECEMBER 2022 // orthotown.comhttp://www.orthotown.com
