when I was there. We, the residents, had to do everything: hand-traced all ceph fi lms, trimmed
models, called the patients, presented and sold the ortho cases to our patients, made all the ortho
appliances, cleaned the lab, etc. I didn't like doing all of these when I was there; now, I want to
thank my program director for making me doing all these because I learned a lot.
My brother's orthodontist, whom I respect and admire greatly, charged my parents only
$2,600 for my brother's treatment (an 18-month treatment with RPE). To my parents, whose
combined incomes were only $20k a year, this was a huge fi nancial commitment for them, even
at such ridiculously low fee. My parents could only afford ortho treatment for my brother; I had
to pay for my own ortho treatment when I was a fi rst-year dental student at UCLA. We were
very grateful for the wonderful job this orthodontist did for my brother. That's how I decided
to start up my own ortho offi ce that targets mainly low-income families like my parents'. Just
last month, my dad saw this orthodontist at a fund-raising concert. My dad told him, " My son
specialized in ortho like you because he always admires your success. " ■

Does the 18%-22% include payroll taxes? ■
In my practice, I do not split out payroll taxes as something separate in the overhead category
of staff salaries. I have nine FT and I have them at about 20% collections. ■

I have always been able to keep my payroll including taxes, 401(k), scrubs, trips, bonus and
payroll expenses under 20%. ■
