Do you have any facial photos to help determine
where the upper midline is located? Is the upper
midline off to the left? If it is, you may be able to get
away with extracting and its premolars with only
some lower IPR. n

Because the 8s are really not an issue, a Forsus like
appliance may be an option. n

From the Panorex, I would guess that the upper
midline is centered. Therefore, extraction of UR4,
UL4 and LL5 would be the most sure path forward
to a centered lower midline, compensating for the
mandibular asymmetry. This would also correct the
moderate dental protrusion. n

Take a current facial photo at a better angle. My
guess from the mid-treatment intraoral photos is
that the maxillary midline is good, but you don't
have enough information right now.
Once you check midlines-consider whether or
not you could extract two maxillary premolars and
a lower incisor. n

The midline in upper is okay now. I extracted the
U4s. n

I understand the dilemma of the right Class II. However,
considering the patient's poor oral hygiene, prolonged
treatment due to the extraction of teeth can
also pose a problem with decalcification & caries. I
hate to say it, but the patient's oral hygiene may limit
you to getting the occlusion to " the best that you can "
instead of " ideal. " n

I agree with Diane's plan of U4s and LL5. Non-extraction
is an option if lip incompetence, peri-oral
strain, instability and a compromised periodontium
are desired treatment goals. n
