If you do move forward, I think what Mike said
is some good wisdom. Let them drift and the L8s
erupt before you put anything in the mouth. Now,
you do have to watch for the bone to start narrowing,
though. This gave me another thought, which
would be to have the L6s hemisected and the distal
half removed and allow the drifting to start. That is
probably the smartest way to go if you start it. n
apicos? Keeping what he has for as long as possible
seems like the best thing. When they ultimately fail,
hopefully, he'll be able to have immediate implants.
Maybe the hemisection idea would work, but the
pathology would have to be taken care of anyway.
If you can find a surgeon who will do transplantations,
please let us all know who it is. And thank you,

My worry about letting L7s drift and L8s erupt
would be that U6s would also have the opportunity
to overerupt. So, I like the idea of a hemisection
more. But I would tell the parents to crown the lower
6s and when need be, implants later in life. n

I think you are setting yourself up for failure. That
will be extremely hard to do and will cost more in
ortho than the implants. It would be great if you
could do it, but I just don't think the GP understands
how hard that will be orthodontically.

If you can find the right surgeon, you could
consider transplanting the lower 8s into the lower
6 position, or perhaps trying only on the left side
where the 6 is becoming painful.
Peter Reich, White Birch Oral Surgery, Dover, NH,
has done a number of bicuspid transplants for me.
So far, no failures. n

before OJ suffers, get 8s in, then stop. If they aren't
happy, then crown the 7s to finish, or leave some
space and see if it closes later in life. I have found
that a small space catches more food than one that
is wide open. n

It would be a big NO for me to even attempt to
drag those teeth forward. I have a terrible time
getting TADs to stay stable in the mandible, and
you would need them for a very long time to get this
done. Years.
I assume the teeth were hypocalcified from Day 1.
Obviously, they should have done crowns soon
after the endos; not sure who thought amalgams
were adequate in this situation. Looks like there's a
radiolucency around the mesial root on LR6. Have
these teeth been evaluated by an endodontist for

One could always consider transplanting the 8s to
the 6 spots. Very high success rate if done properly,
and roots of 8s are not at a bad stage of development.
Should need minimal ortho if any after the surgery. n
