How is she mandibular recessive? SNB is 80.9 and
B-point seems to be pretty on with a TVL through
Nasion. She has a deep bite and an overjet of about
2 mm. Advancing the mandible would place her
into an anterior crossbite. What is the rationale for
a Phase I in this patient? I was unable to find any
scientific basis for or research on Healthy Start. n

The 3 will usually pop out the facial or just stay
right under the surface. If it stays under the surface,
once you OCS to open space, it will begin to erupt. n

Fen, I agree 100%. It's always amazing to me how
difficult we make things. KISS principle: Keep it
simple, sir. After the E's exfoliate, full treatment to
correct anterior remaining crowding. n
I don't see any indication for interceptive treatment.
There's no advantage to extracting tooth R. I consider
the deciduous teeth natural space maintainers,
and in this case, we need to conserve as much space
as possible. If there was residual space in tooth M
position, I would recommend a lower holding arch,
but in this case, I don't see the need. In my practice
this would be a six-month recall, leading to nonextraction
full treatment when only the E's remain.
As for the airway issue, I think it's premature and

Agree with Jhawk-no treatment now. No need to
treat early loss of lower left #M. Extracting #R will
only increase the overset and not really correct the
midline. Wait until able to do a full phase. It is easy
to open space for #22 once you start treatment later.
I would monitor the eruption of maxillary canines
with X-rays every four months or so. Most likely she
would be a full, non-extraction in my practice. n
overkill to start recommending cone beam imaging
and sleep studies. I think that misses the mark and
it's only a distraction from the orthodontic evaluation.
This 10-year-old probably came to you for a
simple ortho opinion on the missing LLc. n

I believe use of LLA to maintain leeway space for
Phase I is all I'd advise. Can shift the midline when
commence with comprehensive care at later time. n

Thanks for posting this. This is a good example of
the cases I am having trouble planning. For those
who observe for one phase, what is the typical
outcome of the erupting LL 3&4? Because the 3
typically erupts first, do you ever observe ectopic
eruption? I want to do as little Phase I treatment as
possible without " errors of omission. " n

Fen, I know you study the literature intently. I try
as well, so this is the reason for my question: Do you
have any evidence that removing the opposite
canine will result in midline correction? I cannot
find any, but do find evidence of arch length loss
without something like an LLA. n
