I wouldn't only focus on the canine. What is your
comprehensive plan for this young lady: Are you
keeping all those teeth, given the proclination,
etc.? If extractions are planned, then give great
consideration to extracting the canine rather than
the bicuspid. n

Non-extraction is not a treatment plan. It is a
decision made based on the diagnoses made after a
careful review of the records.
I'm guessing there was no calculation of crowding
made for this case. This child has a bimaxillary dental
protrusion that warrants extraction treatment.
With that information, you could have planned
which teeth to extract, and the mechanics required
to treat the case. Diagnose the case. n

Not to get too much off-topic, but I've noticed a
strong movement toward non-extraction on the
Facebook orthodontic forums, particularly among
the younger docs.
It's a bit cut off from the images posted, but I really
like the close-up 3D views of the bone around the
lower anteriors. I feel like that is a very helpful indicator
of long-term stability and recession. If I were to
see very thin bone around the lower anterior, I would
lean more toward a four-bi extraction, or in this case
three bis and the impacted canine. n

I've done it both ways before over the years. In this
instance, I'd expose the canine, attach it to the premolar
and get an archwire through that lateral with
a bracket positioned to overcorrect the root position
to the mesial. There's room to transpose the roots
right now, but I'd get a move on before that canine
descends more! n

I think it really depends on the group ... a couple
of the more popular ones are appallingly bad when
it comes to nonsensical and biologically unsound
theories. n
