OF INFORMATION PRACTICES
This Notice of Information Practices explains how we collect, use and protect your personal information and your rights regarding
that information.
INFORMATION WE COLLECT
During an insurance underwriting process, personal information from various sources may be considered. Sons of Norway may
request and obtain information regarding any Proposed Insured's mental and physical health, general character, finances, occupation,
driving records, other insurance coverage and hazardous activities. We may need to collect or verify information from
physicians, medical practitioners, hospitals, clinics, other medical or medically related facilities, insurance carriers, or reinsurance
companies, the MIB, LLC, pharmaceutical records databases, consumer reporting agencies, employers, public records, financial
sources, agents, friends, neighbors and associates. Information may be collected through written correspondence, telephone or
personal contact. Investigative Consumer Reports prepared by insurance support organizations may be retained by those organizations
and disclosed to other entities for which it performs services.
INFORMATION WE DISCLOSE
Information we collect about you will not be given to anyone without your consent, except when necessary to conduct our business.
There are some disclosures which may be made without your prior authorization. These include our field representatives,
third parties, such as persons or organizations that perform professional or insurance functions for us, insurance support organizations,
other insurance companies to which you may have applied or treated medical professionals, judicial process or government
regulatory authority. Pursuant to the law, we may be required to provide law enforcement or other governmental authorities with
information.
HOW WE PROTECT YOUR INFORMATION
We restrict access to personal information to only those individuals who need to know that information to provide products or
services to you. We maintain physical, electronic and procedural safeguards that comply with legal standards and ensure the confidentiality
of personal information in accordance with our policy.
HOW CAN YOU REVIEW YOUR INFORMATION
You have the right to request a copy of the personal information that we have about you. If we receive such a request, we will
provide you with a copy of your requested personal information within 30 days, as long as the information is reasonably locatable
and retrievable. We may charge a reasonable fee to cover the costs insured to provide you with copies of the requested personal
information.
You have the right to correct, amend or delete personal information we may have recorded about you. To exercise these rights,
you must send us a written request. Upon receipt of your request, we will review the recorded personal information and respond to
your request within 30 days.
These rights do not extend to information collected in connection with or in reasonable anticipation of a claim, or civil or criminal
proceeding or to specific items of privileged information when an applicant or policyholder is suspected of fraud, material misrepresentation
or material non-disclosure.
