Progress Measurement You and your mental health professional will continue to assess progress in therapy to ensure you are on track to meet your therapeutic goals. Progress in therapy may be measured by: * Feedback Informed Treatment, which includes objective assessment measures * Whether you act in a manner that is consistent with your goals * How consistently you meet the guidelines and expectations * Evidence that you develop and maintain positive relationships * Continuing focus on your health and well-being * Your ability to move from clinical care to self-care * Whether you follow through on therapy recommendations Discharge Planning You and your mental health professional will determine the appropriate pace and timing of your discharge from therapy, congruent with the goals of your treatment plan. In the event that you or your treatment team believes you are not making the expected progress, the mental health professional will discuss this with you. New treatment recommendations could be made that include a referral to another Hazelden Betty Ford service or even a program outside of Hazelden Betty Ford. If such recommendations are made, you and those individuals you had indicated you wanted involved in your treatment, such as family members, significant others, counselor, physicians and insurance companies, may be included in the treatment and/or discharge planning process. 13
