Building Your Care Plan-
Treatment Programs and Services
Your care plan at Hazelden Betty Ford integrates the latest
research and proven-effective practices-and is carefully
designed to meet your specific needs, with whatever therapies,
mental health services, and medications are appropriate.
Assessment & Evaluation
Our clinicians will conduct tests to examine or reassess
your substance use. If, based on the results, treatment
is needed, your care team will design the most-effective
individualized care plan for you and will discuss
recommendations with you, your family, and others
involved in your care.
Day Treatment Care
As a part of our comprehensive continuum of care, our
day treatment services provide clinical care, structure
and support following your primary residential treatment
to fully support your early recovery. Day treatment will
allow you to begin practicing and demonstrating the
living and life skills learned in primary care in a highly
structured, supportive environment. In this genderinclusive
program, patients participate in individual
counseling, group process, educational and therapeutic
programs, wellness and individualized experiential
workshops, and personal interactions with peers. Your
length of stay is based on your clinical needs, along with
achieving clinical and behavioral milestones.
It has been proven that patients who fully participate
and follow the recommendations of their individualized
care plan have the most-effective outcomes and
sustained sobriety.
Intensive Outpatient Program
Our one-counselor model of intensive outpatient care
is designed to help patients gradually transition from
clinical management to self-management.
Self-management is the ultimate goal when a patient is
thriving in a life of recovery with a low risk of relapse.
Our Intensive Outpatient Program is a blend of clinical
management and self-management. This level of care
provides patients with varying degrees of structure
and support based on their individual clinical needs.
Your initial care plan includes treatment programming

four days a week. As you achieve clinical and behavioral
milestones, your treatment programming will gradually
transition to one day a week. And, as you progress
through treatment, your care team will work with you to
continually determine the most-effective level of care.
In outpatient care, patients participate in individual
counseling, group process, educational programs, and
personal interactions with peers.
Your length of engagement is based on your clinical needs,
along with the amount of time it takes to reach clinical and
behavioral milestones. Evidence suggests that patients
who fully participate and follow the recommendations
of their individualized care plan have the most-effective
outcomes and sustained sobriety.
Typical treatment activities
* Educational Services: Our educational component
features workshops presented by staff members, as well
as video and printed materials.
* Group Process: A topic will be introduced and
discussed at the beginning of each group process
session. The group will then use the remaining time for
group process, which is a specific form of facilitated
group discussion.
* Individual Intensive Counseling: Individual
counseling allows you to meet one-on-one with your
counselor and work on your individualized care plan.
Individual counseling sessions may include:
- Cognitive-behavioral therapy
- Motivational interviewing
* MyRecoveryCompass: is your secure online resource
providing access to several helpful tools, important
information, and supportive content for your early
recovery:
- Communicate with the care team through a
personalized message center.
- View your group and individual appointments.
- View your individualized plan of care (treatment plan).
