Building Your Care Plan-
Mental Health Services
- View your Patient Summary (continuing care plan).
- Complete feedback-informed treatment (FIT)
assessments. These brief and research-based
questionnaires focus on topics important to your
care and treatment. The results will help your care
team better understand your progress, guide your
course of treatment, and respond quickly to clinical
needs and potential red flags in your recovery.
- Utilize MOREĀ®, My Ongoing Recovery Experience,
which includes recovery workshops, assessments,
videos, an extensive resource library, and more.
Services
Addressing mental health concerns is an important part
of getting well and staying well for many patients. Our
mental health services are available on an outpatient
basis to support your individualized care plan as needed.
Whether you are just beginning your recovery journey,
have celebrated many milestones, or are a family member
working your own recovery plan, our outpatient mental
health services are designed to meet your needs. Our clinic
provides the following services on an outpatient basis:
* Group therapy
* Individual therapy
* Substance use assessment
* Family therapy
* Families in Recovery Group
* Seeking Safety for Women's Group-Maple Grove
* Couples therapy
* Opioid support groups
Special Programs
You have access to mental health clinicians to support
your individual needs, which may include:
* Anxiety
* Depression
* Trauma and PTSD therapy
* Family dynamics
* Other co-occurring disorders
Program Expectations
Appointments
Your front desk staff is happy to help you schedule your
mental health sessions. Call 952-679-2936 for Chaska
and 763-401-5890 for Maple Grove. Due to the high
demand for services, it is important to arrive on time
for all appointments or call ahead if you need to cancel
your appointment.
If you do not provide a 24-hour notice for your
appointment cancellation, you will be charged a noshow
fee. Insurance will not cover charges for missed
appointments/late cancellation and you will be
responsible for the payment. To avoid this fee, please call
952-679-2936 for Chaska and 763-401-5890 for Maple
Grove at least 24 hours in advance of your scheduled
appointment if you need to cancel or reschedule.
Medications
If you are using psychiatric medications, please
continue to work with your health care provider as
needed. Please communicate to your counselor any
psychiatric medications you are currently taking and
sign a release so we can communicate with your health
care provided as needed. A General Designation Release
will be requested for coordination of care.
Emergencies
For after-hour emergencies, contact your county mental
health crisis line or emergency department by calling 911.
Confidentiality
Your privacy is important. No information about you will
be shared outside of Hazelden Betty Ford without your
signed consent except in the case of life-threatening
emergencies. Please note that mental health clinicians
are required by law to report suspected cases of abuse
toward children and vulnerable adults.
Billing
Payment is due at the time of your appointment unless
other financial arrangements have been made with your
financial advocate. Your copay or coinsurance is always
due at the time of service. If an insurance claim is to be
filed, we must have complete and accurate insurance
information, including a copy of your insurance card as
well as a signed Release of Information for Payment and
Reimbursement in order to properly file an insurance
claim. If your insurance or financial status changes,
please advise the staff member at the front desk.
You have the right to examine and receive an
explanation of your bill, regardless of the source of your
payment (cash, insurance, or third-party payer). Please
contact your financial case manager or patient accounts
representative to help answer any questions you have.
