Facility Abuse Prevention Plan
Chaska-2023
Introduction
Minnesota law identifies some adults as vulnerable and therefore
susceptible to abuse. This designation may apply to certain individuals with
substance use disorders and to certain others by reason of mental illness
or physical disability. This document fulfills the Minnesota Department
of Human Services requirement for a plan to prevent abuse of persons
served by Hazelden Betty Ford Foundation in Chaska. The plan is derived
from assessments of the population served, the physical plant, and the
environment.
Assessment and Plan
Assessment of the Population
The outpatient substance use disorder programs at Hazelden Betty Ford
Foundation in Chaska accepts people who experience problems relating to
substance use and/or mental health conditions for treatment. Adults ages 18
or older are served. Individuals accepted for admission are capable of selfcare,
communicating their needs, of acting on their own behalf and in their
own interest, and of understanding and exercising their rights, including the
right to file grievances. All prospective patients are screened for violence
and sexual offense history as well as other clinical history elements that
may increase abuse risk. Level III sex offenders are excluded by admission
criteria.
Pre-admission screening procedures determine the extent to which
individuals meet admission criteria. Individuals are screened for physical
limitations that may render them inappropriate for programming structure.
Individuals are also screened for mental, emotional and cognitive limitations
that may render them inappropriate for programming structure. Assessments
identify maladaptive behavior and/or the presence of other conditions, such
as known violent crime or acts of physical aggression toward self or others,
which may place the patient and others at risk. Individuals not meeting
admission criteria are not admitted to the program, however referrals may be
provided.
Individuals with a history of substance use disorders are susceptible to
continued use and, while using substances, may experience diminished
ability to exercise caution in relations with others and act responsibly in
their own interest. Withdrawal concerns may exacerbate existing physical,
emotional and mental distress. Patients may also have greater vulnerability
due to clinical factors such as history of abuse, cognitive limitations, poor
personal boundaries, impulsivity, difficulties with assertiveness, etc. The
workforce member recognizes these factors in the prevention of abuse. A
multidimensional assessment of each individual is made to determine the
extent to which they may be susceptible to abuse during the current course
of care as well as for continuing care. Thereupon, appropriate measures,
including referrals, are incorporated into the individual's care plan.
Chaska's therapeutic programs require active involvement of patients,
notably in the development and implementation of individual treatment
plans. The workforce member subscribes to those therapeutic methods
with a high degree of involvement of the individual. Patient participation
is expected, and family and other support system participation is sought,
when appropriate, in the development of individual treatment plans.
Mechanisms of data collection include phone calls, family conferences, and
questionnaires. Workforce safety surveys are also conducted periodically to
identify safety gaps and concerns.
General orientation is provided to all patients. Each individual receives a
handbook at the time of admission. Rights and responsibilities, as well as
behavioral expectations are detailed and made available to each individual
and or their representative on request. Additionally, copies of the list of
patient rights, the grievance procedure, and telephone numbers of State
regulatory and advocacy agencies, this abuse prevention plan, and other

pertinent documents are posted in a designated area accessible to patients
and other visitors at the Chaska site. Each patient's status as a vulnerable
adult is assessed at the time of admission and patients are encouraged to sign
the Maltreatment Reporting Release Form. Patients may voice their concerns
at any time in individual appointments with workforce.
Assessment of the Physical Plant as Relates to Safety of Patients
All patient service areas are in excellent condition and meet applicable
codes. Building construction complies with MN state license regulations. Our
accrediting and licensing agencies routinely inspect buildings and grounds
for conformance with their safety standards. Service areas are contiguous
and buildings are designed to promote safety through therapeutic interaction.
Access to the building is via two entrances, one that is a primary entrance,
the other through the building cafeteria. Access to the clinic space is through
a single entrance, which is monitored by administrative support workforce
members during clinic hours. The clinic's hours of operation are Monday
through Thursday from 8 a.m.-9 p.m. and Friday from 8 a.m.-4 p.m. There
are multiple exit doors throughout the building, including emergency exits.
There are three exit points within the clinic space, including two designated
emergency exits into the corridor hallway outside the clinic space.
All service and business areas are locked when not in use and monitored
to prevent unsupervised access. Areas of the building that are difficult
to supervise include public restrooms, the elevators and other common
areas of the building. Through the facility's ongoing hazard surveillance
program hazards to safety are identified and corrected. Incident reports
are investigated promptly, and safety processes are modified when needed
based on the findings. Emphasis on therapeutic peer relationships promotes
safety by fostering interpersonal awareness and supporting disclosure of
untoward events. A variety of activities routinely occur to assure the wellbeing
of patients. These include resolution of concerns, support of peers and
workforce in resolution of complaints or grievances, incident reports, use
of questionnaires and other reporting mechanisms to elicit information on
patients' experiences in treatment.
Assessment of the Environment
Hazelden Betty Ford Foundation in Chaska is an Outpatient Substance
Use Disorder and Mental Health clinic located in a multi-tenant building.
This program is located in a commercial neighborhood with nearby private
businesses, including a golf course, retail shopping and restaurants. The
terrain of the golf course includes ponds and areas that are not visible
from the clinic. Measures to minimize risk include communicating clear
expectations to patients and addressing any indications of inappropriate
behavior. During service hours, patients are instructed to remain within the
clinic space or on the floor of the suite. If a patient does not return from a
break as expected in a timely manner, a search is conducted in the clinic to
locate the patient. If the patient is not located, workforce attempts to contact
the patient via phone call.
The suite within the building has one entry point and multiple exits. It
contains 4 group rooms, 11 offices, 1 private bathroom, and a kitchenette.
The treatment programs maintain staffing patterns and utilize professional
workforce in accordance with the needs of those served and the services
provided, and with requirements of licensure. Staffing is sufficient to allow
for appropriate monitoring of patients and response to any emergency
situations. A primary counselor is assigned to each patient to assure
adequacy and continuity of care. Workforce having direct contact with
patients are subject to the state of Minnesota's mandatory background
check. Workforce are trained in the Vulnerable Adult law at time of hire
and annually thereafter. Workforce also receives additional training to deescalate
crisis situations.
None of the programs use procedures that might potentially inflict physical
or mental harm, for example, restraints or seclusion, electro-convulsive
therapy, psycho-surgery or other surgical procedures, use of painful stimuli
for behavioral modification, use of unusual or investigational drugs, or
research procedures that involve inconvenience or risk.
