Outpatient Services
Individual counseling
You will be assigned a substance use counselor who
will work closely with you and your family on a weekly
basis. You will formulate a treatment plan together and
review your progress in your individual sessions.
Group therapy
Group therapy is facilitated by a substance use
counselor and is designed to help patients work on
various issues. You will be asked to share your personal
story with peers and to complete other treatment plan
assignments. These sessions allow you to explore issues
in greater depth and to receive feedback and support
from other people in recovery.
Spiritual care
Substance use affects the body, mind and spirit.
Spiritual care is an essential component of each
patient's recovery and is offered through educational
opportunities during the program. Our spiritual
philosophy respects the belief systems of each patient.
Hazelden Betty Ford is not affiliated with a religious
denomination.
Family involvement
Hazelden Betty Ford has various support offerings for
families, caregivers, and loved ones. Third party contact,
with a valid release of information, will occur on set
intervals based on the level of care you participate in, as
well as on an as needed basis. Third parties are provided
clinical updates, as well as partnered with in continuing
care planning.
Hazelden Betty Ford offers a Virtual Family Program at
no cost, and a pay for service in-person Family Program
option at our Plymouth campus.
Bibliotherapy
Educational materials may be assigned to you by your
counselor. Copies are available in the program at no
charge; however, you may also purchase your own
copies to use at anytime via HazeldenBettyFord.org/
Bookstore.

information. Our books, workbooks, journals, pamphlets
and DVDs are designed to help you strengthen your
recovery and focus on moving forward. You will
find information about all of Hazelden Betty Ford's

We have a bookstore on campus in Plymouth. Please
work with your counselor to obtain materials.
Outpatient Mental Health

Mental health concerns affect your judgment, emotions,
beliefs and behavior. That's why our treatment approach
is holistic and personalized, providing care for your
mind, body and spirit-and taking into account
everything you need to get well.
The following section provides information about our
services and describes each step you will take in your
recovery at Hazelden Betty Ford. If you have additional
questions, your mental health professional will answer
them.
Clinical Services
In outpatient mental health, we provide a menu of
services to assess and treat the mental health needs of
individuals and families. Mental health professionals
assess and treat based on a bio-psycho-social model
of intervention through the creation of individualized
treatment plans.
Mental health assessment
Mental health services begin with a thorough
assessment to determine the appropriate care for each
individual. The assessment process includes structured
clinical interviews and self-report questionnaires.
Mental health assessments are provided by highly
educated and qualified staff.
