of maltreatment, they shall report as soon as possible,
but in no case longer than 24 hours, using either the
internal or external reporting protocols. When the
reporter uses the internal protocol, the facility has 24
hours from date/hour the original report was made to
fulfill the external reporting obligation. Documentation
must clearly indicate the dates/hours of reports.
Maltreatment of minors
All incidents of suspected maltreatment of minors
will be reported if a patient discloses/reports alleged
abuse or neglect of any minor in any state in the past
three years. The report will be made by the clinician
who became aware of the reportable situation, and the
mandated report will be completed within 24-hours
or less. If a child is believed to be in danger, local law
enforcement will be contacted immediately.
It is not required to obtain patient authorization to
submit an initial report of alleged child abuse or neglect;
however, a release of information is required for any
additional follow-up requesting new information after
an initial report.
The mandated report will be submitted to Child
Protection in the county where the alleged abuse

Reporting Maltreatment of Minors
Receiving Care in a Licensed
Facility
Reports of minors who are suspected of being abused
or neglected in a facility licensed by the Department
of Human Services are to be made to the Minnesota
Department of Human Services (DHS) Maltreatment
Intake line at (651) 431-6600.

Prenatal exposure to controlled
substances
Reporting is mandatory if a pregnant person has
engaged in habitual or excessive use, for a nonmedical
purpose of a controlled substance for nonmedical
purpose during pregnancy or any of the following
substances or their derivatives (260E.31, subd. 1):
* Opium
* Cocaine
* Heroin
* Phencyclidine
* Methamphetamine
* Amphetamine
* Tetrahydrocannabinol
* Alcohol
It is not required to obtain patient authorization to
submit the initial report, but is required for follow-up
after the initial report.
Reports will be submitted by the clinician who first
became aware of the reportable situation to Child
Protection in the patient's county of residence. Local

Crime(s) committed on the premises of a Hazelden
