Financial Exploitation Risk:
In general, clients admitted to the facility do not have access to significant
financial resources due to their age and ongoing dependence on others for
financial support. Thus, the primary area of risk for financial exploitation
exists between clients to the extent that they may exert pressure on each
other to borrow or be given spending money and other personal property
while in the facility. Clear expectations are communicated regarding the
prohibition on loaning of money and exchanging of personal property
between patients. Workforce are also fully trained on the law prohibiting
financial exploitation and on the corporate policy regarding Professional
Patient Boundaries.
Contraband/Use in Treatment Risk:
Contraband
If/When an individual is determined to have a history of bringing
contraband into a facility in previous treatment episodes, or is otherwise
assessed as at risk for contraband, measures will be taken to minimize the
potential for safety risks associated with contraband and the resulting risk of
atypical discharge.
* All clients/patients entering the facility for programming will be asked to
submit to a search of their person and belongings. (This includes Day/IOP
clients entering the building on a daily basis.)
* Program workforce are trained on search procedures, what is identified as
contraband, and appropriate disposal and/or storage of contraband items.
* Program rules and expectations regarding contraband will be explained
to each program participant. This will include a definition of what
contraband consists of, the rationale for why it is considered contraband,
and procedures used to ensure contraband does not come into the facility.
All patients sign a consent to search as part of the Admission and Consent
Agreement.
* Program rules and expectations are also reviewed with family members
and concerned persons to ensure that they are aware of what is considered
contraband, the rationale for not allowing it on the premises, and a list
of acceptable items that they can bring for their young person, or send
to them while they are in the facility. (Done prior to admission whenever
possible.)
Substance Use in Treatment
All clients/patients entering Hazelden Betty Ford in Plymouth are oriented
to the policies and procedures regarding the implications should someone
use a mood altering substance while under Hazelden Betty Ford's care, or on
our premises. The following information will be reviewed with each patient/
client entering one of the treatment programs at the Plymouth location.
* No mood altering substances are allowed on these premises for any
reason.
* If substances are discovered they will be appropriately disposed of.
* Rapid Urine Drug Screens and breathalyzers will be administered as
needed to determine if someone has been using mood altering substances
while in care.
* Workforce are trained to observe patient/client behaviors to include sight
and sound indicators that may suggest someone is under the influence,
and appropriate steps to take should they have such a concern.
* A Risk Factors ad-hoc form is conducted on each client entering one
of our programs, to include histories that would inform us if there have
been past instances of using while in treatment. This tool is designed to
identify those who have a history of using while in a treatment center,
and the clinicians assigned to the care of such individuals will implement
treatment planning to avoid substance use and atypical discharge.
* All patients/clients who have been identified as using opiates, are given
a detailed *Opiate Warning/additional resources to describe the risks
associated with overdose or potential death should they return to use.
* Procedures are in place to respond to instances of actual substance use
by patients. Additional detail regarding these response procedures may be
found in the site's Impairment/Under the Influence procedure.
Impaired Ability to Communicate Risk:
Patients/Clients entering into our care are screened for vulnerabilities
through a variety of tools, to include collateral information collected from
schools (IEP's), previous health care providers, and family/concerned
persons. When it has been identified that a client entering or in our care,
struggles with the ability to communicate vulnerabilities they may be
experiencing, individualized care plans will be implemented to ensure that
tactics are put in place to facilitate communication in a manner specific to
the client's needs.
* A treatment plan will be developed with input from the client, and all
stakeholders who may have relevant information regarding limitations of
the individual.
* This treatment plan will be reviewed no less than one time per week by
the multi-disciplinary treatment team to ensure that the areas of concern
are being adequately addressed and meeting the needs of the patient/
client.
* Advocacy for this type of client is essential, therefore frequent progress
updates should be conducted with the patient's/client's family or assigned
guardian.
* Assistance with communication using available technology and/
or interpreters may be used when indicated or any other reasonable
accommodation
Impaired Judgement/Difficulty Understanding and Following Therapeutic
Direction Risk:
Some individuals admitted to the facility have difficulty understanding
and/or following direction from workforce. Underlying factors for this risk
may include intoxication, short and long term cognitive consequences of
substance use, co-occurring medical or mental health conditions, lack of
therapeutic alignment/limited motivation for change and developmental
factors. Such individuals may also have limited recognition and awareness
of safety hazards, limited ability to protect themselves in unsafe situations,
have difficulty asking for assistance and are more likely to intentionally or
unintentionally expose themselves to unsafe situations and to take risks.
