Using Information for Improvement
Hazelden Betty Ford utilizes Press Ganey surveys for
patient feedback (refer to Performance Excellence in
Clinical Services). Staff at Hazelden Betty Ford attempt
to incorporate feedback into process improvements.
Designated staff members at Hazelden Betty Ford
participate in Best Practices team meetings, which
focus on identifying areas for improvement and are
accountable to making these changes for ongoing
improvement in the organization.
Quality, Safety & The Joint
Commission
Individual Hazelden Betty Ford facilities are accredited
by The Joint Commission, which sets standards for
quality and safety of patient care. If you have concerns
about safety or quality of care provided, please contact
leadership at Hazelden Betty Ford so that your concerns
may be addressed by calling 866-831-5700.

Patient Rights
Department of Public Health Title 22 (Detox,
Residential, Medication Services)
(a) The governing body shall adopt and implement a
written policy on patients' rights.
(b) A list of these patients' rights shall be posted in
English and in the predominant language of the
community if other than English, in appropriate
places within the CDRH so that such rights may be
read by patients.
This list shall include but not be limited to the patients'
right to:
(1) Considerate and respectful care, including the right
to wear appropriate clothing.
(2) Knowledge of the name of the physician who has
overall responsibility for their care, the names and
professional relationships of others who will be
involved in their recovery program.
(3) Be free from discrimination based on sex, race, color,
religion, ancestry, national origin, sexual orientation,
disability, medical condition, marital status, or
registered domestic partner status.

(Outpatient)
Each client shall have rights that include, but are not
limited to, the following:
1. The right to confidentiality as provided for in HIPAA
and Title 42, Code of Federal Regulations, Part 2.
2. The right to be accorded dignity in contact with staff,
volunteers, board members and other persons.
3. The right to be accorded safe, healthful and
comfortable accommodations to meet his or her
needs.
4. The right to be free from verbal, emotional, physical
abuse and/or inappropriate sexual behavior.
5. The right to be informed by the program of the
procedures to file a grievance or appeal discharge.
6. The right to be free from discrimination based on
ethnic group identification, religion, age, gender,
race, sexual orientation, or disability.
7. The right to be accorded access to their file.
8. The right to take medications prescribed by a
licensed medical professional for medical, mental
health, or substance use disorders.
