Uncovering More Truths With 3D Data
A Cone Beam Computed Tomography (CBCT) scan was taken to help determine the etiology of the pain and to
determine the appropriate treatment plan. An ORTHOPANTOMOGRAPH™ OP 3D™ 6 x 9 cm single jaw FOV with the
highresolution protocol (95 kVp, 4 mA, 200 μm) was utilized. This protocol was selected to minimize exposure, yet to
focus on the entire upper jaw. After reviewing the dataset with the 3D imaging software, I was able to truly visualize
the third dimension of the maxilla, slice through the region of interest and gain clarity on the lesions associated with
the patient's pain.

Treatment Plan
Angulating the high-resolution slices allowed me to clearly cut away the superimposed anatomy (that absconded my
view in the 2D panoramic taken at the initial consultation). Superior to tooth #3 there was a thickening of the sinus
lining present along with a fenestration into the sinus (Figure #4a) that was not evident in the 2D periapical x-ray
images.
Furthermore, tooth #14 showed a defined radiolucency surrounding all three roots with missing bone of the buccal
cortex accompanied by a resorptive defect on the distal buccal root surface. (Figure #5a). Upon review of tooth #15, I
observed severe bone loss surrounding all roots and loss of bone levels along the alveolar crest. (Figure #5b)
