Following the Correct Path
2D imaging isn't able to show the entire picture. Our CBCT helped reveal the etiology of the patient's bilateral maxillary
posterior pain. As a result of the differential diagnosis that 3D imaging provided, we canceled the endodontic and
periodontal referrals. The third dimension allowed us to see that we needed to refer the patient to our oral surgeon.
With proper collaboration, we have a treatment plan which includes extractions and bone grafts. After some time to
heal and promote bone growth, we will place implants in sites for teeth #3, #14, and #15. Upon writing this clinical
case, the patient had followed through with extractions, was in a healing process and planning to have implants
placed soon afterwards.

Conclusion
3D data provides a level of clarity above and beyond the limitations of 2D intra and extra-oral imaging. In this
clinical case, although the periapical lesions on teeth #14, #15 and #3 were evident with our digital 2D x-rays, the
ORTHOPANTOMOGRAPH™ OP 3D CBCT scan data allowed for a more comprehensive diagnosis in that it provided
the clarity needed to determine the size of the lesions, uncover the resorptive defects, visualize the extent of bone
loss, and communication with the sinus. This holistic view not only allowed us to determine the most appropriate
treatment plan, but also to focus on timing of the treatment.
Additionally, the images helped our patient confidently understand the extent of the issue. This was a major factor in
convincing this recalcitrant patient of the severity of the issues, the need to act, and to ultimately accept our treatment
plan.

Dr. Francis LaCouvée attended the University of Victoria and completed three years towards a microbiology and cellular physiology degree and was granted early admission to the UBC Faculty
of Dentistry. Simultaneously, Francis enrolled in the Dental Officer Training Program with the Canadian Military.
Upon graduating from the four-year dental program, Francis was posted to Lahr, West Germany, as a One Canadian Division dentist from 1986-1990, with a final posting to Petawawa, Ontario,
with the Canadian Airborne Regiment.
After completing his military service, Dr. Lacouvée began private practice in Qualicum Beach on Vancouver Island, British Columbia. In 1998, his dream to build his own practice came to fruition
