Recovery Enhancement:
An Innovative Neuroscience
Based Treatment to Help Heal
the Opioid Crisis

Opioid misuse among people with chronic pain
fueled the current opioid epidemic. However, to
date, there are no evidence-based treatments that
simultaneously address chronic pain, opioid misuse,
and opioid use disorder (OUD). To meet this need,
Dr. Garland designed mindfulness-oriented recovery
enhancement (MORE) through a decade-long
treatment development process funded by the
National Institute on Drug Abuse. Rooted in affective
neuroscience, MORE unites complementary aspects
of mindfulness training, CBT, and positive psychology
into an integrative therapy to target the brain
reward system and reduce addictive behavior, stress,
and chronic pain. This session will detail the theory
underlying the MORE treatment approach, research
data demonstrating MORE's effectiveness, and
unique clinical skills integral to this novel treatment
approach. To date, MORE has demonstrated efficacy
in four clinical trials involving 490 patients. Across
these studies, MORE has been shown to decrease
opioid misuse twice as effectively as standard psychological
therapy, while simultaneously reducing
pain, craving, and depression. At the same time,
MORE increases brain responses to natural rewards,
healthy pleasure, and meaningful life experiences.
Given these findings, MORE should now be disseminated
by addiction treatment and behavioral health
providers throughout the country to help stem the
tide of the opioid crisis.

125. Sexual Boundary Violations By
Professionals

Sexual boundary violations among healtcare professionals
is a serious ethical, moral, and usually illegal
event, with impacts that are devastating to all parties.
The recent and unprecedented barrage of individuals
coming forward with evidence of past sexual
coercion, misconduct, and criminal abuse has shown
a spotlight on the underreporting of boundary violations
in many professional relationships and settings.
Yet there continues to be inadequate empirical investigation
and evidence on the prevalence of sexual
assault and misconduct in the healthcare professions,
especially among psychotherapists. This, along with
a lack of consensus on the basic question of what constitutes
boundary violations among disciplines and
in various jurisdictions, leaves information gaps that
can be filled only with more rigorous examination,
prevention efforts, reduction of reporting barriers,
and implementation and development of treatment
strategies for both parties. In this presentation,
attendees will review risk and protective factors while
gaining an understanding of the assessment, treatment,
and monitoring process for behavioral health
professionals who have engaged in this egregious
behavior. This issue is critically important on several
levels, but none more so than the emotional injury inflicted
on vulnerable individuals seeking professional
help. Clinicians will be provided specific examples to
maintain ethical compliance.
126. Differential Diagnoses and
Treatment of Mental Health and
Substance Use Disorders

LEVEL OF INSTRUCTION: ADVANCED
This presentation will cover the differential diagnosis
of the most frequently seen mental health disorders
(Anxiety, Depressive, Bipolar, and Schizophrenia
Spectrum/Psychotic disorders) and the various substance
use disorders (opioid, alcohol, amphetamine,
etc). We will examine the overlap and relationship
between disorders and symptoms, examine the criteria
for each, and look at how family or past history
can help or hinder diagnoses. Particular attention
will be paid to changes in symptoms and differential
diagnoses as the patient progresses through detox
and early treatment. We will also briefly examine
the definitions, symptoms, and overlap of the less
frequently diagnosed disorders (Paraphilic, Somatic,
Sleep-Wake, Dissociative Disorders, and others).
We will then discuss treatment options for these
disorders including medications and psychosocial
services as well as appropriate levels of care. Finally,
we will look at the warning signs indicating a possible
return to substance use or increased psychiatric
problems and ways to prevent a recurrence or relapse
in these disorders. Along the way, we will compare
these relapse rates and predictors of relapse for SUD,
psychiatric, and other medical disorders such as
hypertension and type 2 diabetes.
