Initial training in attitude instrument flight is usually conducted at
altitudes well above any nearby obstructions. Once the student progresses
to departure, arrival, and approach procedures, each flight follows courses

carefully designed to assure clearance from both towers and terra
firma. Self-interest and professional obligation alike give the
instructor powerful incentives to make sure the aircraft doesn't
stray too far off track.
Aside from the odd mechanical problem, there wouldn't appear to
be all that much to go wrong. That's why the actual record came as
something of a shock. While preparing our recent review of Accidents
During Flight Instruction (airsafetyinstitute.org/accidents-duringinstruction) the Air Safety Institute took a hard look at the types of
training being carried out in the 615 fixed-wing accidents in which
the pilot under instruction already held at least a sport certificate.
