AFTER REVIEWING THE ACCIDENTS IN DEPTH, CERTAIN COMMON
THEMES POPPED UP. NOT SURPRISINGLY, MANY LOC ACCIDENTS
OCCURRED WHEN PILOTS FLEW IN AIRCRAFT OR CONDITIONS THAT
WERE WELL BEYOND THEIR SKILL LEVEL.
flying at night when not night current, and flying a new aircraft
with little or no experience in its type. The findings resulted in
recommendations for improving aeronautical decision making and
setting personal minimums.
Some LOC accidents, which resulted from poor decision making
based on improperly set personal minimums, were pretty obvious
from the start. For example, there was a Piper Aztec accident,
where the non-instrument-rated private pilot took off into an
overcast sky with cloud bases observed at 200 to 300 feet agl
(NTSB Identification LAX06FA289). We can conclude with some
certainty that this pilot flew beyond his capabilities.
In other accidents, the line used to determine go/no-go decisions
was not black or white but very grey. Here's one example: A
Cessna 182 lost control after encountering IMC on a night flight
from Scottsdale, Arizona. The pilot was a flight instructor holding
a commercial certificate and instrument rating. The passenger was
an instrument-rated private pilot. In this instance, the pilot had only
gotten four hours of sleep the previous night, flew to Scottsdale with the
intention of spending the night, but decided to fly back in the middle
of the night (NTSB Identification SEA07FA199). Although instrument
current, he had only a total of about 8.7 hours of actual instrument time.
It's unclear in the accident report what weather briefing the pilots
received before flight, but certainly they anticipated a VFR flight at
8,500 feet on a very dark night, with high overcast skies over a sparsely
populated area where there would be very little visual references.
Given the circumstances, the limited experience, and the fatigue, this
accident serves as a good conversation starter when discussing personal
minimums objectives.
Personal minimums should provide a buffer between what is legal and
what is smart. Air carriers have rules in place to mandate minimums
based on particular circumstances. If a new crewmember has less than
